AMPBELLSVILLE, KY (WAVE) - The Campbellsville Fire Department has been fined $25,200 by Kentucky workpace safety regulators for violations related to an ALS Ice Bucket Challenge that killed one firefighter and injured three others.
Captain Tony Grider died 30 days after the Aug. 21 accident at Campbellsville University where members of the fire department were helping the university's marching band with its Ice Bucket Challenge.

The Kentucky Office of Occupational Safety and Health determined that the fire department filed to ensure that the firefighters wore protective helmets designed to reduce electrical shock hazard when near exposed electrical conductors.
Furthermore, the agency found that the fire department did not ensure that the firefighters wore footwear designed to protect from electrical hazards and falls.

The department was also cited because the firefighters were not trained and familiar with safety-related work practices, specifically safe approach distances to overhead power lines such as those they came into contact with during the Aug. 21 accident.
Grider and one other firefighter were in the bucket portion of an aerial ladder truck when the accident happened. The other two firefighters who were shocked were on the ground near the truck.
